Safety Investigation Training
Course Overview:
Within an organisational safety culture, many incident/occurrence reports are generated.
Operators, Service Providers and Training Organisations are mandated by European Regulations to investigate such reported occurrences. The quality and effective output of these investigations is fully dependant on the competence and training of the safety team/investigators.
This course will give your safety team/investigators an understanding of:
- How investigations are conducted at a state Safety Investigation Authority (SIA) level;
- The inter-action between the SIA and the entity that is the subject of an SIA investigation;
- The technical and investigative understanding/knowledge about how to conduct investigations in such a way that the organisation will learn from these events and reduce the likelihood of similar events occurring in the future.
By means of discussions, case studies, video/practical examples, and group exercises, participants will become familiar with:
The objectives of a Safety Investigation;
- The Roles and Responsibilities within a Safety Investigation Team
- Human Factors from four different angles;
- The development of dealing with Human Error;
- The required Terms of Reference for Investigators;
- The value of Safety Investigation Interviews;
- The different approaches to Data Analysis;
- The Collaboration with other Investigating Authorities.
- Dealing with Families/next-of-kin;
- Dealing with the media.
An optional exam will be available on completion of this course.
In additional, an optional day course is available (on completion of the 3 Day course) that will introduce the delegates to the practical aspects of report writing and they will conduct a practical report writing exercise.
Learning Objectives:
On completion of this course the delegate will have attained sufficient theoretical and practical knowledge to allow them to:
- Become familiar with the basic process of a safety investigations
- Practise all aspects of a safety investigation
- Generate awareness about the Human Factors related to safety investigations
- Perform a small scale safety investigation as part of team,
- Be in a position to inter-act with state SIAs in the event that they are the subject of an investigation.
